Transition planning is a process mandated by the Individuals with Disabilities Education Act (IDEA 2004) for all youth who have an Individualized Education Program (IEP) in K-12 education. The purpose is to identify the youth’s goals for after high school and create a plan to help them achieve those goals.

Planning For The Future

The Planning for the Future Checklist is a great place to begin. The checklist is designed to support students with disabilities, as well as their families and IEP team members, to ensure they are reviewing all aspects of transition planning at age appropriate intervals.
