Pathways to Partnership (P2P) is a multi-year project funded through a Disability Innovation Fund (DIF) grant from the U.S. Department of Education. The project brings together partners across Pennsylvania to improve how youth with disabilities prepare for life after high school.

Pathways to Partnership uses a coordinated approach to improve transition outcomes for youth with disabilities across Pennsylvania. The work is organized into four connected areas.
Training and professional development help educators and youth service professionals build shared understanding and skills related to transition. This includes Charting the LifeCourse frameworks and tools, the Transition Systems Framework, and other offerings coordinated through PaTTAN and partners.
Regional teams use the Transition Systems Framework to organize predictors linked to positive post-school outcomes and to guide local systems work. This supports more consistent, evidence-informed practices across schools and agencies.
Centers for Independent Living collaborate with local OVR offices and schools to provide community-based instruction and paid work experiences. These opportunities give youth real-world practice in work and community settings while still in school.
The PA Secondary Transition website is being refined with age-appropriate materials for youth and expanded resources for professionals, families, and employers. These enhancements make information easier to find, understand, and use.
The Office of Vocational Rehabilitation was awarded funding from the Rehabilitation Services Administration through the U.S. Department of Education to implement Pathways to Partnership.
